The Rapid Assessment Team at Lynn’s Queen Elizabeth Hospital won an award at the Norfolk Community Health and Care NHS Trust Recognition of Excellence and Achievement in Community Health Awards. The team took home the trophy for Clinical Partnership Working. The team is made up of nursing staff employed by Norfolk Community Health and Care, therapy staff employed by the trust and social care staff employed by Norfolk County Council. Sara Tansley, rapid assessment team leader said: “I know what we do at the QEH not only benefits our patients but also our colleagues. Patients receive the care and support they need in order for them to remain as independent as possible, through a variety of services. It was fantastic to win, we’ve been over the moon ever since!”
